Closing the Alzheimer Disease Clinical Care Gap by Creating Local Therapeutic Communities Independent of Socioeconomic Status
Montefiore Einstein has a long and distinguished history of integrating state of the art and emerging research in Alzheimer’s Disease with unique models of community engagement and partnership in historically complex and diverse, but also vulnerable populations. We are leveraging our expertise to reduce service fragmentation and increase equitable access to community-based support services and clinical trials.
This has been recognized with a five-year grant renewal from the NYS Department of Health. This grant will allow us to build upon our signature “5Cs” paradigm: Culturally Competent Collaborative Care of the Cognitively Impaired. At the forefront of culturally competent collaborative care, research and community, our state-designated Montefiore Einstein Hudson Valley CEAD (Center of Excellence for Alzheimer’s Disease), continues to reduce the dementia care gap by creating local ‘therapeutic communities’ that neutralize the stigma of socioeconomic disparities. The Montefiore Einstein Hudson Valley CEAD, is one of ten such centers of excellence for Alzheimer’s Disease in New York State
